DENTSPLY LUMINAS SWEEPSTAKES OFFICIAL RULES

How to Enter: a. You must fully complete the DENTSPLY Club Membership Registration Form including providing your full name,
mailing address(es), telephone number(s) and e-mail address(es). You will automatically be entered when you, before the deadline
indicated below, submit a fully completed DENTSPLY Club Membership Registration Form. b. To be considered all entries must be fully
completed, received by May 1, 2008. Mail-in entries must be post-marked no later than April 30, 2008. c. Only one entry per individual
respondent. d. This Sweepstakes is only open to U.S. graduating dental students.
Additional Rules: a. The DENTSPLY Luminas Sweepstakes prize will be awarded to one winner. The winner will be determined in a
random drawing on May 9, 2008. Persons entering the contest give their permission to use their names and photos for publicity
purposes without compensation. Winner will be notified by mail and phone and will be required to submit an affidavit of eligibility and
sign a release form, both of which must be returned within 14 days of the date they are mailed to the potential winner. In the event the
prize goes unclaimed or winner fails to meet all requirements to collect the prize, the winner will be chosen in a second chance random
drawing of all other eligible entrants, following these same rules. b. The DENTSPLY Luminas Sweepstakes prize will be awarded to
one individual who will receive one complimentary registration for the DENTSPLY Practice Success for the New Dentist program, June
21-23, 2008. The cash value of the prize is up to $2,100 USD. This prize cannot be substituted for cash and is non-transferable. Complimentary
registration includes: course tuition, meals, functions and activities offered in conjunction with the meeting, two nights (June
21 & 22, 2008) hotel accommodations (single occupancy) at a hotel chosen by DENTSPLY, and airfare up to $600. In order for the
airfare to be paid for by DENTSPLY, flight must be booked through DENTSPLY Travel Services at 1-800-877-0020 x54479. Hotel incidentals
are the responsibility of the prizewinner. c. No purchase is necessary to enter. d. Odds of winning are dependent upon the
number of entries. e. Taxes on the prize are the responsibility of the prizewinner. f. DENTSPLY International is not responsible for lost,
mutilated, or misdirected entries. g. For the name of the prize winner, send a self-addressed stamped envelope after May 31, 2008 to:
